Die Ökonomie besteht aus einer unvorstellbar großen Zahl solcher Bausteine. Jede Branche nutzt ein Subset davon - jedes Unternehmen kombiniert daraus sein eigenes.

Was Software heute in Unternehmen tut.
What software does to companies today.
- Sie digitalisiert, damit automatisiert werden kann. Jeder Bereich, der in Software abgebildet ist, wird messbar, wiederholbar, automatisierbar. Das ist der Gewinn - und der Grund, warum wir alle sie einsetzen.
- Aber sie wird von Menschen gebaut, die sie selbst nie benutzen. Von Menschen mit einer Vorstellung davon, wie andere arbeiten sollten - das ist der eigentliche Grund für alles Folgende.
- Deshalb zwängt sie ein. Weil Code teuer ist, sucht jede Standardsoftware den gemeinsamen Nenner möglichst vieler Betriebe - und schreibt dem einzelnen Unternehmen vor, wie es zu funktionieren hat.
- Die Großen kaufen sich den Ausweg - und selbst der ist ein Korsett. ERP-Systeme wie SAP sind längst ein Baukasten: Module, Customizing, Integratoren. Individuell wird es nur für die, die Millionen und Jahre investieren - Lidl brach 2018 nach rund 500 Mio. € ab, an einem einzigen fehlenden Baustein (Einstandspreise statt Verkaufspreise).
- Mehr Funktionen helfen nicht. Softwarehäuser bauen immer mehr ein; benutzt wird ein Bruchteil - weil niemand den Rest versteht.
- It digitizes so things can be automated. Every area captured in software becomes measurable, repeatable, automatable. That is the win - and why we all use it.
- But it is built by people who never use it themselves. People with an idea of how others ought to work - that is the real root cause of everything that follows.
- So it forces you into a corset. Because code is expensive, every standard product targets the common denominator of as many businesses as possible - dictating how each company has to operate.
- The big players buy their way out - and even that is a corset. ERP systems like SAP have long been a construction kit: modules, customizing, integrators. It only becomes individual for those investing millions and years - Lidl abandoned its rollout in 2018 after some €500m, over a single missing block (purchase prices instead of retail prices).
- More features don't help. Vendors keep adding functionality; a fraction gets used - because nobody understands the rest.

PraxisPlus ist unsere eigene Ordinationssoftware - der Versuch, die ersten drei Schichten für eine Hausarztordination wirklich zu bauen. Möglich wurde das erst, weil KI den Code schreibt. Was fehlt, ist die vierte - dazu gleich.

PraxisPlus is our own practice software - the attempt to actually build the first three layers for a family practice. It only became possible because AI writes the code. What's missing is the fourth layer - more on that in a moment.

Unsere Ordination: eine Kassen-Allgemeinordination. Die klassische Arztsoftware (CGM) bedient die regulierten Bausteine - ELGA, Kassenabrechnung, Registrierkasse. Alles andere läuft in einer eigenen Software: PraxisPlus - seit Anfang Februar 2026 im Echtbetrieb. Mit dem Livegang hat sie jedes Fremdsystem ersetzt, das wir bis dahin genutzt haben.
Our practice: a public-insurance family practice in Vienna. The traditional medical software (CGM) covers the regulated blocks - e-health records, insurance billing, fiscal receipts. Everything else runs on our own software: PraxisPlus - in production since early February 2026. At go-live it replaced every third-party system we had used until then.

Nicht aus Frust. Weil zwei Dinge fehlen - und keines davon ist ein Feature.
Not out of frustration. Because two things are missing - and neither is a feature.
Fehlt 1Der Kompositeur - die vierte Schicht.PraxisPlus bildet ab, wie wir heute arbeiten: schneller, sauberer, messbar. Aber es bildet ab. Individuell wird dadurch nichts. Erst der Agent macht aus dem Baukasten ein eigenes Angebot für einen einzelnen Menschen.
Fehlt 2Das Geschäftsmodell - und das ist das Größere.„Individualisierung von allem" heißt nicht individuelle Software. Es heißt: individuelle Patientenreise, individuelles Programm, individuelle Ansprache, individuelle Arbeitsweise von Ärztin und Team, individuelle Preise. Ein Unternehmen, das das leisten soll, muss von A bis Z darauf gebaut sein - und die Software ist dann sein digitales Abbild, nicht sein Ausgangspunkt.
Missing 1The composer - the fourth layer.PraxisPlus mirrors how we work today: faster, cleaner, measurable. But it mirrors. Nothing becomes individual through it. Only the agent turns the kit into an offering of one person's own.
Missing 2The business model - and that's the bigger one."Individualization of everything" does not mean individualized software. It means: an individual patient journey, an individual program, individual communication, an individual way for physician and team to work, individual pricing. A company meant to deliver that must be built for it from A to Z - and the software is then its digital image, not its starting point.
Wichtig: PraxisPlus steht nicht still. Es trägt unseren Betrieb jeden Tag und verbessert sich über den Feedback-Loop von Folie 5 laufend selbst weiter - das bleibt so. Was endet, ist meine Weiterentwicklung: Die nächste Stufe baue ich von Grund auf neu, weil sie mehr verlangt als ein Update - denn PraxisPlus war nie das Produkt. Es war der Prototyp des Systems.
Important: PraxisPlus is not standing still. It carries our practice every day and keeps improving itself through the feedback loop from slide 5 - that continues. What ends is my development of it: I am building the next stage from scratch, because it demands more than an update - PraxisPlus was never the product. It was the prototype of the system.

Ein Geschäftsmodell braucht ein erstes Produkt - unseres ist die Vorsorge. Heute gibt es dafür genau ein Angebot: die eine Vorsorgeuntersuchung im Jahr. Dasselbe Programm für alle, eine Momentaufnahme, kein Plan.
A business model needs a first product - ours is preventive care. Today there is exactly one offering for it: the one annual check-up. The same program for everyone, a snapshot, not a plan.

Der Unterschied - und die Grenzen
Nicht dasselbe Programm für alle, sondern für jeden ein eigenes - das gab es bisher nicht, weil es nicht bezahlbar war. Und es bleibt ärztlich: Jede medizinische Entscheidung trifft die Ärztin, nie das System; die Medizinprodukte-Grenze (MDR) wird vor jedem Start geprüft; Gesundheitsdaten bleiben im Haus (DSGVO Art. 9). Was die Kasse zahlt, läuft über die Kasse.
The difference - and the guardrails
Not the same program for everyone, but one of each person's own - which never existed, because it wasn't affordable. And it stays medical: every clinical decision is made by the physician, never by the system; the medical-device boundary (MDR) is checked before any launch; health data stays in-house (GDPR Art. 9). Whatever public insurance covers goes through public insurance.

Die Hypothese: Business as a Service
Franchise war immer das Gegenteil von Individualität - überall dasselbe. Mit einem Baukasten und einem Kompositeur wird beides zum ersten Mal gleichzeitig möglich: ein System für alle, ein Betrieb für jeden. Jeder Partner baut aus demselben Baukasten seine eigene Kombination, seine eigenen Abläufe.
